Sports Physicals & First Practices
Athlete must fill out a physical form and have a parent signature in order for a doctor to see them. Students without a filled out form or parent signature will be turned away. The normal cost if $25.00 and will be performed by local doctors from Rockford Orthopedic and Associates and Swedish American Health Systems.
